Visiting Nurse Service of New York Clinical Specialist, Nurse Practioner in Brooklyn, New York

OVERVIEW
Provides advanced nursing clinical care to patients in the Hospice/Palliative Care program. Works in collaboration with hospice physicians and other hospice team members to develop policies and clinical protocols for the Program. Provides clinical leadership and instruction to others within Hospice in quality care, consistent with agency philosophy and standards of nursing practice in relation to Agency goals and objectives. Works under general direction.
RESPONSIBILITIES
* Reviews new referrals with the Interdisciplinary Team to determine suitability and eligibility for admission based on the admission criteria of the program and policies. Recommends and helps with care planning.
* Assists management in managing patient care; answers questions and provides guidance to field staff.
* Provides advanced nursing care and treatment to hospice patients, which includes assessment, diagnosis, prescribing medication, ordering and interpreting appropriate tests, and ordering treatments and DME.
* Provides leadership to members of the interdisciplinary team. Acts as a subject matter expert to clinical staff in the assessment and treatment of patients and families.
* Collaborates with patients, families, primary care physician and other team members to provide assessment and care planning. Consults with the primary care physicians and specialists in the community, when necessary, in relation to management and disposition plans.
* Identifies the need for new/revised clinical protocols. Collaborates with Hospice Medical staff and others within Hospice to research and develop new/revised protocols for specialty programs including but not limited to cardiac care, COPD care, pain management, and wound care. Continues to evaluate program(s) and makes recommendations for improvements, as appropriate.
* Participates in developing and delivering continuing education programs for clinical staff regarding newly established/revised clinical protocols for effective patient care services.
* Acts as subject matter expert in designated specialty and provides guidance to new/current employees and MD/NP fellows for complex care management situations.
* Provides Palliative Care Consultations to patients/families as necessary.
* Assesses patients at F2F visits when necessary for certification at admission to the Hospice Program and re-certification at specified intervals for continued service in the program. Completes certification of death.
* Travels to multiple regions, visiting patients/families, and serves as the primary caretaker for patients/families without medical supervision.
* Documents patient care services in accordance with VNSNY standards. Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided.
* Directs staff nurses on treating complex patient care issues.
* Covers team in absence of management; reassigns staff as appropriate.
* Provides care in accordance with VNSNY policies, practices, procedures and Standards of Nursing Practice, which may require standing, stooping, sitting, couching, bending and stretching to deliver patient care.
* Provides on-call coverage, as needed.
* Participates in special projects and performs other duties as assigned.
QUALIFICATIONS
LICENSURE: Licensed and currently registered to practice as Nurse Practitioner in New York State required. DEA license required. Valid driver's license may be required, as determined by operational/regional needs.
EDUCATION: Master s Degree in Nursing required.
EXPERIENCE: Minimum of five years clinical nursing experience including two years in the specialty of advanced illness management, oncology and/or palliative care or hospice required. Minimum of two years as a Nurse Practitioner preferred. Experience in a community health setting, conducting professional presentations and writing for professional publications preferred.
Demonstrated knowledge of symptom management, understanding of end of life issues with appropriate interventions, and understanding of the impact of cultural background on illness management required. Demonstrated knowledge of home care and/or hospice services and regulations, Title XXII, Medicare Conditions of Participation, JCAHO home health and/or hospice accreditation requirements, QI outcomes/benchmarking, infection control, reimbursement patterns for Medicare and other payers, and budget and other financial reporting required.
