Full-time, 35 hours/week - $24/hour

The LPN/Case Manager Assistant Responsibilities
Monitors and maintains medication area:
Monitors and maintains proper storage of medications at residence.
Ensures that expired medications (including OTC) and treatments are disposed of properly.
Follows controlled substance policy for counting, storage, administration and disposal.
Verifies medications are properly labeled, correspond correctly to prescriber's order and the MAR.
Performs weekly medication counts and order medications from pharmacy.
Ensures the overall cleanliness of the medication administration area and assists the RN with infection control of this area.
Schedules MD and other clinical appointments as directed by the RN:
Creates consults to communicate with provider and ensures proper documentation is sent to provider as directed.
Attends appointments with individual as needed.
Communicates with RN regarding the outcome of appointment (new orders, med changes, recommendations, etc.)
LocationLiving Resources Corp
The LPN shall be licensed and currently registered by the New York State Department of Education. At least one-year experience working with the developmentally disabled, is preferred.
Valid NYS Driver's License required.
