The FCPS Transition Specialist works with individuals age 18 and older with intellectual and other disabilities including mental health and physical disabilities to develop an individualized and person -centered letter of Intent. This is accomplished by incorporating the individuals' circle of support including caregivers into the interview process. The FCPS Transition Specialist is responsible for screening potential customers for FCPS and completing necessary referral and enrollment paperwork. The FCPS Transition Specialist works on a team of five employees and provides general office coverage and support to ensure efficient operations and delivery of quality customer service. The FCPS Transition Specialist must be familiar with and demonstrate a working knowledge of benefits and services available to individuals transitioning out of the school system as well as individuals who are aging out of the workforce and transitioning into retirement.
DUTIES & RESPONSIBILITIES
1.Coordinate Letter of Intent (LOI) interview/ service meetings with individuals and his/her identified circle of support
2.Facilitate Letter of Intent (LOI) interview process and develop, individualized and person -centered LOI's which are then incorporated into the comprehensive future care plan.
3.Screen potential new FCPS customer referrals, coordinate initial meeting, and facilitate FCPS program enrollment for signed on customers.
4.Conduct post plan survey and provide follow-up phone consultation/ support with individuals/ care providers.
5.Facilitate appropriate referrals to service providers.
6.Be FCPS's ambassador: Represent our services in a positive and professional manner when engaging with customers and other community professionals. Conduct FCPS/AADD program overview presentations and outreach to a variety of community providers.
7.Complete required paperwork, data collection and reports within required timeframes
8.Participate in all required trainings and certifications
9.Other assigned duties
EDUCATION: Bachelor Degree in Human Services required. Social Work degree preferred.
EXPERIENCE: Minimum one-year experience working with people with Intellectual and Developmental Disabilities Working knowledge of OPWDD Eligibility and services including Self Determination, Broker Services and benefits including MEDICAID, Medicare, Social Security required. Working knowledge of OMH Eligibility, Benefits and community resources preferred. CQL- certification and working knowledge of personal outcome measures (POM's) preferred.
