Job Summary:Acts as liaison between state, national and international calling physicians and all UH clinical faculty, professionals and staff. New/current patient referrals from physicians (by phone, fax, email/website): determines which physician is most appropriate, directly schedules appointment, when possible, facilitates via department if not -- provides patient and physician correspondence, acquires necessary medical records, and obtains insurance authorization. Coordinates telephone consults between physicians and tracks records to ensure follow up as needed. Gathers pertinent clinical information as specified by clinical areas. Coordinates urgent appointments and schedules same day ancillary radiology appointments. Outbound Activity: Completes daily admission notification faxes to PCP's (when patient has signed the necessary release of information forms). Provides medical records to referring physician when patient follow-up in PCP office prior to records received from Clinical Data. Schedules inpatient discharge follow-up appointments prior to patient discharge. Makes outbound calls to recently discharged patients who completed phone survey to help with follow-up appointment scheduling and inquire about patient satisfaction during their stay. Customer Service Skills - Answers incoming phone calls per standards set by Telephone Doctor training series Maintains professional tone with callers (internal and external) at all times. Problem-solve to ensure referring physician's needs are met or exceeded. Backs up Upstate Connect main line as needed.

Minimum Qualifications:

Associate's degree and 2 years of relevant health care experience (preferably in an outpatient or physician office setting) or equivalent combination of education and experience plus exceptional written/verbal communication and customer service skills, ability to handle multiple tasks and computer skills required.
