Job Description

Assesses, plans, implements and evaluates the educational needs of individual diabetes patients, family and support systems regarding diabetes self-management skills. Utilizes empowerment theory, motivational interviewing, goal setting and teach-back evaluation in a patient-centric approach to education. Conducts diabetes self-management education according to best practices in both individual and group setting. Coordinates care, collects diabetes data, and serves as a diabetes resource to healthcare team.

Responsibilities Include:

  1. Assess and teaches diabetes self-management education skills including:
  • Blood glucose monitoring

  • Insulin action, timing, action and administration

  • Medication education

  • Hypo and Hyperglycemia signs, symptoms and treatments

  • Exercise guidelines

  • Stress Management techniques

  • Post-partum follow up guidelines

  • Insulin pump therapy

  • Continuous Glucose Sensor

  • Reduction of risks

  • Survival skills

  1. Assesses nutritional needs and provides nutritional counseling to assigned clients/patients.
  • Assesses nutritional education needs of patients enrolled in Diabetes Education Care on an individual basis. Considers patients cultural, educational and learning issues.

  • Plans and provides individualized nutrition education to patients, their families and or significant other based on their needs assessment.

  • Documents nutrition information discussed, written materials provided and evaluation of patient's receptiveness and comprehension of the education provided.

  • Provides group nutrition education classes.

  • Provides follow up nutrition education to patients/participants.

  1. Assists in planning, implementation and evaluation of the objectives and curriculum related to:
  • Type 1 and Type 2 Diabetes mellitus

  • Pre-Diabetes

  • Continuous insulin infusion therapy

  • Gestational Diabetes Mellitus

  • Diabetes prevention

  • Obesity/weight loss counseling

  • General nutrition needs

  1. Assesses, documents and helps coordinate plan of care for appropriate measures including:
  • HbA1c/ Glucose screening tests

  • Dilated retinal eye exams

  • Nephropathy Screening

  • EKG

  • Foot Exams

  • Patient goal setting

  1. Responsible for data collection utilizing appropriate documentation tools.

  2. Communicates patient information to referring providers and appropriate staff in a timely manner.

  3. Participates in professional meetings and conferences.

  4. Performs related duties, as required.

Qualifications

  • Bachelor's Degree in Food, Nutrition or Dietetics, required.

Registered Dietitian, required.

OR

Bachelor's Degree in Nursing, required.

Current license to practice as a Registered Professional Nurse in New York State, required.

  • Current Diabetes Educator Certification, required.

  • Minimum of three (3) years progressively responsible, related experience to ensure familiarity with scope of program.
