Assesses, plans, implements and evaluates the educational needs of individual diabetes patients, family and support systems regarding diabetes self-management skills. Utilizes empowerment theory, motivational interviewing, goal setting and teach-back evaluation in a patient-centric approach to education. Conducts diabetes self-management education according to best practices in both individual and group setting. Coordinates care, collects diabetes data, and serves as a diabetes resource to healthcare team.
- Assess and teaches diabetes self-management education skills including:
Blood glucose monitoring
Insulin action, timing, action and administration
Hypo and Hyperglycemia signs, symptoms and treatments
Stress Management techniques
Post-partum follow up guidelines
Insulin pump therapy
Continuous Glucose Sensor
Reduction of risks
- Assesses nutritional needs and provides nutritional counseling to assigned clients/patients.
Assesses nutritional education needs of patients enrolled in Diabetes Education Care on an individual basis. Considers patients cultural, educational and learning issues.
Plans and provides individualized nutrition education to patients, their families and or significant other based on their needs assessment.
Documents nutrition information discussed, written materials provided and evaluation of patient's receptiveness and comprehension of the education provided.
Provides group nutrition education classes.
Provides follow up nutrition education to patients/participants.
- Assists in planning, implementation and evaluation of the objectives and curriculum related to:
Type 1 and Type 2 Diabetes mellitus
Continuous insulin infusion therapy
Gestational Diabetes Mellitus
Obesity/weight loss counseling
General nutrition needs
- Assesses, documents and helps coordinate plan of care for appropriate measures including:
HbA1c/ Glucose screening tests
Dilated retinal eye exams
Patient goal setting
Responsible for data collection utilizing appropriate documentation tools.
Communicates patient information to referring providers and appropriate staff in a timely manner.
Participates in professional meetings and conferences.
Performs related duties, as required.
- Bachelor's Degree in Food, Nutrition or Dietetics, required.
Registered Dietitian, required.
Bachelor's Degree in Nursing, required.
Current license to practice as a Registered Professional Nurse in New York State, required.
Current Diabetes Educator Certification, required.
Minimum of three (3) years progressively responsible, related experience to ensure familiarity with scope of program.
