Westhab, Inc. - Yonkers HUD VASH CASE MANAGER in Yonkers, New York

SUMMARY Under the supervision of the Social Work Supervisor for, the case manager coordinates the case management services for veterans receiving housing provided through HUDs Section 8 housing vouchers.

DUTIES & RESPONSIBILITIES:

. Performs site visits of HUD VASH apartments to ensure that veterans reside in safe environments in compliance with local housing codes.
. Actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on programs procedures or policy changes based upon work experiences.
. Must have knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services and ability to coordinate services.
. Must be able to independently assess the psychosocial functioning and needs of veteran and their family members and to formulate and implement a treatment plan, identifying the clients challenges, strengths, barriers, coping skills and assistance needed in collaboration with the client, family and interdisciplinary treatment team.
. Must be able to independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds.
. Must have knowledge and experience in the use of medical and mental health diagnoses, disabilities and treatment procedures.
. Must be able to provide consultation services to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
. Must have knowledge and skills in the use of computer software applications for drafting documents, data management and tracking, especially those programs in use by the VA
. Participate in weekly team case conference meeting or as deemed appropriate
. Perform other job related duties as assigned.

EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:

Master's Degree and at least 5 years of demonstrated case management experience with high needs/high intensity of services populations. MSW preferred. Fluency in Spanish is highly desirable. Ability to work some evenings and occasional weekends. Must have own car and a valid NYS drivers license required
